Paula is a highly experienced RN with over 20 years of expertise in medical chart review, patient safety analysis, and quality management. With a strong background in both hospital and post-acute settings, Paula specializes in identifying medical errors, assessing standards of care, and detecting evidence-based harm in patient medical records.
Paula leads the Medical Records Review team which identifies patient harms, trains nurses in medical chart analysis, and ensures accuracy in clinical data abstraction. She has extensive experience using the IHI Global Trigger Tool (GTT), a proven method for identifying and measuring adverse events in healthcare.
Her expertise also includes fraud, waste and abuse detection. Throughout her career, she has worked closely with physicians and regulatory agencies to provide critical insights into medical necessity, appeals, and healthcare compliance.
Paula holds a compact nursing license, allowing her to practice in multiple states under the Nurse Licensure Compact (NLC). This enhances her ability to collaborate on multi-state medical cases and provide expert medical chart reviews across jurisdictions.

Dr. Miller is a board-certified internist with a focused practice in hospital medicine. After obtaining his undergraduate degree from the University of Pennsylvania in 1991 he matriculated to Tufts University School of Medicine where he received his MD in 1995. Following a transitional internship at Alameda County Medical Center in Oakland, California from 1995-1996, Dr. Miller worked as a general practitioner in California and Alaska. He completed a Master of Public Health at the University of California in 1998 and subsequently completed his residency in Internal Medicine at the University of California, Los Angeles in 2001. Since completing his training, Dr. Miller’s clinical practice has primarily been as a hospitalist. He is a Senior Fellow in Hospital Medicine in the Society of Hospital Medicine and has served as president of the Greater Philadelphia chapter. He currently sits on the chapter’s executive board. Dr. Miller is an Assistant Clinical Professor of Medicine at Sidney Kimmel Medical College at Thomas Jefferson University. Administratively, Dr. Miller has served in several roles including director of Associates in Hospital Medicine at Jefferson Methodist Hospital as well as Associate Chief Medical Officer for Jefferson Health. He has led and sat on numerous committees focused on quality and safety within Jefferson Health. Dr. Miller oversees the clinical integrity of our Medical Records Review work.

Julie is a Registered Nurse (RN) with expertise in Medicare Quality Improvement Organization (QIO) program services, healthcare regulatory affairs and policy, project management, utilization management, and managed care. Her expertise includes medical review systems, coding, and DRG (Diagnosis Related Group) validation and quality improvement. She has over 30 years of experience working with state Medicaid agencies and CMS, conducting utilization reviews and quality of care reviews, claims audits for Nursing Homes, Acute and Specialty Hospitals, and conducting external quality reviews for Medicaid managed care programs audits.
She has direct experience developing, testing, and implementing medical review systems and internal quality review programs. As senior vice president of Delmarva Foundation for Medical Care, Inc. (DFMC), Julie won multiple awards for exceptional operations, survey administration, and quality framework. Her recent experience includes serving as the primary consultant for Tyler-MED. In this role she was the QIO subject matter expert for MITRE, working extensively with the Centers for Medicare & Medicaid Services (CMS) and MITRE as the CMS FFRDC contractor.
For the past 6 years, Julie has utilized her experience as a regulatory affairs expert to support Avar Consulting, Inc.’s (based in Rockville, MD) Beneficiary and Family Centered Care Quality Improvement Organization (BFCC-QIO) contract, including serving as the Medicare Regulatory Expert for the National Coordinating, Oversight and Review Center, (NCORC) in which she directed a major QIO improvement effort while providing QIO regulatory expertise and leveraging regulatory experts within CMS.
